Health Insurance Options for Self-Employed Business Owners: Marketplace, SHOP, ICHRA, and QSEHRA

Written by Yarik Yarosh, CPA (US & Canada) September 5, 2026 · FL CPA license AC61704 · CPA Ontario

Health insurance is one of the largest expenses for self-employed business owners, and the tax code provides several mechanisms to make it more affordable. The self-employed health insurance deduction (IRC 162(l)) allows sole proprietors, partners, and S-Corp shareholders to deduct 100% of health insurance premiums as an above-the-line deduction on Form 1040. This reduces income tax but NOT self-employment tax (the deduction is on Form 1040, not Schedule C). For businesses with employees, the Individual Coverage Health Reimbursement Arrangement (ICHRA) and the Qualified Small Employer Health Reimbursement Arrangement (QSEHRA) provide tax-efficient ways to help employees pay for individual market health insurance without the complexity and cost of a traditional group plan.

Which health insurance approach is best?

Is business insurance tax deductible?

Business insurance premiums (general liability, professional liability, property, cyber liability, and business interruption) are fully deductible as ordinary business expenses on Schedule C or the corporate return in the year paid. Workers’ compensation premiums are deductible as a labor cost. Health insurance premiums for sole proprietors and more-than-2% S-Corp shareholders are deductible on Form 1040, not on Schedule C. Key person life insurance premiums are NOT deductible, though the proceeds are generally tax-free. If a policy covers both personal and business use (auto insurance on a mixed-use vehicle), only the business-use percentage is deductible.

What happens when I receive an insurance payout?

The tax treatment depends on what the payout covers. Proceeds that reimburse a previously deducted expense (lost inventory, repair costs) are taxable income because the deduction already reduced tax. Proceeds exceeding your adjusted basis in a destroyed asset create a taxable gain, though IRC 1033 allows deferring the gain by reinvesting in similar property within two years. Business interruption payments that replace lost profits are taxable income. Liability claim proceeds (insurance pays a settlement on your behalf) are generally not taxable to the business. Document every claim and payout for the tax return.

What insurance premiums are tax-deductible for self-employed individuals?

Self-employed individuals can deduct health insurance premiums for themselves, their spouse, and dependents as an above-the-line deduction on Form 1040, which reduces AGI directly. This deduction under IRC 162(l) is limited to your net self-employment income from the business that established the plan, and you can’t claim it for months you were eligible for employer-subsidized coverage through a spouse’s job. Business insurance premiums including general liability, professional liability (E&O), property insurance, and business interruption insurance are fully deductible as ordinary business expenses on Schedule C. Workers’ compensation premiums are deductible as a business expense, and key person life insurance premiums are deductible only if the business isn’t a beneficiary of the policy.
